Instructions for Updating Your Report:
Please click "Let's get started!" and then click "Next" at the bottom of each page until you reach the question(s) that you would like to update your response to.
When you have finished your updates, click next until you reach the final page, and click "Review Answers."
When you have finished reviewing your report and your updates, please click "Submit" at the bottom of the page.
Instructions:
Complete the questions below as fully and honestly as possible.
Before beginning, please make sure you have a copy of your club’s top recipient’s application and references on your local computer—you may need to scan it, download it from the application system, or save it from your email to your computer. You will need to upload a copy of the top applications and references later on this form.
You will be asked to report currency in $USD. Please use this calculator to find your currency's equivalent in US before you start the report.
Keep a copy of the application and completed reporting form for your club's files (a PDF copy of your submitted report will be emailed to you).
Saving Your Report:
There is no need to save the report as you go unless you are going to finish it at a later time. If you do need to save the report, please click the downward triangle next to "How to Save Your Form" at the bottom of each page. Please follow the instructions.
To save and return to this reporting form at a later time, click "Save." You will be prompted to create an account on the online platform which hosts this form, JotForm.
Creating an account is not necessary! Instead, click "Save" then click the text at the bottom of that box in blue which reads "Skip Create an Account."
Please type your email and a link will be sent to you with all of your information saved or you can copy the link and save it somewhere to access it later.
